All started with some ideas which wer drawn on a paper. After deciding for the right direction a biography and a story behind the figure were written. These words should help the developers from Valve to create the Demoman and the angry Scotsman was worked out early.
Biographie des Demos
"A fierce temper, a fascination with all things explosive, and a terrible plan to kill the Loch Ness Monster cost the six-year-old Demoman his original set of adoptive parents. Later, back at the Crypt Grammar School for Orphans near Ullapool in the Scottish Highlands, the boy's bomb-making skills improved dramatically. His disposition and total number of intact eyeballs, however, did not."
Using the biography the designers were creating more concepts and the silhouette of the final Demoman appeared more and more.

After feeling the silhouette alone wasn't distinctive enough further changes were made.

The 3d model was created and designers wanted to make the Demo more interesting. The surprising turn was dropping the red-haired scotsman cliché. An internal suggestion was the matter for making the Demoman an Afro-American.
